Freud defined symptoms as a "compromise solution” as he discovered conditions that manifested that under a symptom which is painful in a level, in another level it could lead as well to a sort of satisfaction. In another words, what bothers the patient, is or has been a "kind of solution” founded in an early stage. In this way it is possible to understand the symptom as an answer, or an invention that the patient does in his encounter with difficulties. To Lacan, the difficulties are always related to the desire of the Other. And this is actually what is at stake when thinking about the treatment. The relation of the patient to the clinician, or what Freud named as the transference, is fundamental as this, in its own condition defines the treatment. The standardized protocols and therapies find its limit here, as this positioning is individual with every subject, and in this place the clinician must invent in order to approach the subject symptoms and desire. In this way it is possible to think the treatment as well as an invention.
Alexandre Stevens gives a perfect example of this stablishing the institution with no standards called L’Courtil that works with autism and psychotic children. It is not what he calls the "paranoic institution which is the normal, where rules apply to one and all, invasive and where we know what is the best for the person”. He founded what he called "the schizophrenic institution which is an institution that objects to the very foundation of the institution. Is an institution which is sufficiently disorganized, and not in the side of the knowledge. It is an institution that allows itself to be divided by the subjects who address it. It is not an institution drive by an ideal, but rather one that helps the child to develop what he is looking for, not being decided in advance. It is in another way, making the institution per subject, not adapting the subject to the institution, but the institution to the subject.”
Alexandre Stevens er psykoanalytiker i Belgia, Psykiater, medlem av le École de la Cause freudienne og the World Association of Psychoanalysis. Han er grunnleggeren til L’Courtil som er en anerkjent institusjon som behandler barn som ikke andre behandlings institusjoner vil ha, p.g.a. vanskeligheter med å håndtere autisme eller psykose. Han har skrevet en rekke artikler i det Lacanianske feltet, og ledet «Eurofedereation of psychoanalysis» for mange år tilbake. Hans arbeid i L’Courtil har blitt vist med filmen «Like an open sky» og boken med samme navn. Filmen har blitt visst på kino verden rundt. Han har også forelest i de fleste europeiske land og har vært en rekke ganger i Danmark invitert av Rene Rasmusen.
